ADVICE/INFO FOR PARENTS AND PARTICIPANTS:
1. Turn in your physicals and emergency forms to your coach EARLY. You may also turn in physicals to the Activities Office but make sure emergency forms go to your coach. The first day of fall try-outs is Monday, 8/4 at 3:00pm in the Big Gym at MHS with an all-sports meeting. Forms must be turned in by that day in order to participate. In order to expedite the process that day and allow for more on-court or on-field play, having your forms turned in prior to that date.
2. Keep a copy of your physical form for later use (camps the next summer, etc). Also, make a copy of your emergency form for each sport you wish to try out for in order to hand it in to each season's coach.
3. These forms can be found at the bottom of this web-page.
