Nurse’s Notes

Parents/Guardian,

Give Kids a smile is hosting free dental clinics on February 10^th and 11^th. Please contact the nurse for an application for this wonderful service.

If your contact information has changed, please inform the office so I can reach you in case your child is ill or has another emergency situation during school hours.

Also, if your child has received immunizations or has a change in medications, conditions, diagnoses, please contact the nurse so we can include this information in your child’s health file.

Remember to dress according for our weather children do recess outdoors until it is 20 degrees or lower.

Also promptly wash and return borrowed clothing so I have it on hand for the next child in need.
