Students will be asked to submit various items during the college application process during their Senior year. Below you will find documents to help guide you through this process.

A timeline for Seniors who are applying for College Admission and Scholarships
Transcript Request
Once the blue sheet is on file, students will complete one of these forms EACH time a transcript needs to be sent. They are also available in the guidance office.
Autobiographical Sketch
This form, to be filled out by the student, is used when requesting letters of recommendation from the College Center.
Blue Sheet
This form has a required parent signature and must be on file before the College Center will send any high school transcripts or test scores for a student.

Parent Brag Sheet
This form, to be filled out by a parent, is requested when a student asks for a letter of recommendation from the College Center. This extra information helps to create a more personalized & comprehensive letter of recommendation.
Recommendation Letter Packet
This packet includes basic instructions for letters of recommendation and includes a wavier to be signed by both the student and parent.
High School Resume Sample #1 and #2
Although resumes are not a required part of a college application, many students may choose to send a resume as a way to highlight their achievements in the scholarship process.
